Purchase Orders

The form to order equipment or any needed item is located on the MME website. Purchase order forms can be found at Purchase Order. This form must be filled out, signed by the club treasurer and advisor. The completed form must then be taken to Jan in the MME office in Sloan 201.

Travel Request Forms

In order to travel as a club (to competition) a travel request form must be filled out and turned in to the MME department. The form can be found at Travel Form. This form must be filled out for each member of the club who is traveling. This can be done on one form, but everyone who is traveling must have their name and student ID number on the form.
