First, Click Create

Second, Give your survey a title

Third, Fill out optional data

Once this data has been entered through the Create New Survey Wizard, it can be edited again from Survey Properties.

1. Description: This is a non required text area to give users a better understanding of what this survey is used for.

2. Goals: Your stated goal about what you want to get out of this survey. Not required.

3. Begin/End Dates: Tells the system when to allow access to take this survey. All surveys must have these dates.

4. Delivery Mode: There are 4 options here: (See Learn more )
